Questions to keep beside you

Ask for an answer you can write down, then ask what it means for your choice.

About the health review

Who will review my health, and what is their professional role? Where can I check that the clinician’s permission to give care covers my state location at visit time?

What health information will the doctor or other health professional need? How can I add something the form sent before my first visit did not ask? Will I get a chance to raise my questions before you make a decision?

If a test or an in-person visit is proposed, who arranges it? Who reads the result and explains what happens next?

About the medicine and its source

What is the full medicine name? Is this medicine supplied as an injection, a tablet, or something else? What health problem is it meant to treat? If this medicine has FDA approval, can you show me its label as reviewed by that agency, the Food and Drug Administration?

If it is made to order, what personal medical need explains that choice? Can you name the pharmacy that makes it and the one that ships it? Where can I check the pharmacy's state record?

Who will explain the label and answer a question about what arrives? How do I report a package that does not match the order?

About payment and later care

What does the care fee include? What is charged separately? What is the ongoing total after any starter offer ends?

When do charges renew? How do I cancel? What refund terms apply? What changes if I commit to a longer period?

Who handles care questions after the visit? How does a review work? How can I obtain my notes if I move to another care service?

Keep unanswered questions beside the written offer. Ask for an answer before agreeing to a plan.
