Arrange the visit
Get your questions ready
A short page of notes can hold the things you do not want to forget.
Write down why you are asking
Start with your question in your own words. Are you trying to understand an offer, discuss a health concern, or compare ways to get care? Leave the answer open.
Bring a list of the medicines and supplements you use. Copy their names from the labels. Add the health details you want to discuss. Mark anything you are unsure about instead of filling a gap with a guess.
If you have an existing care plan, bring your questions about how the proposed care would fit beside it.

Make the conversation practical
Choose a place where you can hear and speak. Keep the service's instructions for joining the visit nearby. Ask ahead how to send a document or add a detail after the visit.
If an offer uses words you do not know, copy those words into your notes. Ask the clinician to explain them in ordinary language. Ask what each term changes about your choice.
Ask: What will you need to know before deciding whether this care fits me?
Leave space for the next step
The state medical boards’ national group advises the clinician to do any further testing needed to understand your health or arrange it, including a referral to someone who can perform the tests.
Write down who handles that step. Ask how the results will be explained and what decision follows them. Finish with an answer you can repeat in your own words.

This is advice that state medical boards can use when making rules. Your state's rules decide what the clinician must do.
